

Provider Pathway is the key to saving money on all your long-term care needs. Just one phone call provides you access to a full range of options at discounted rates, making care more affordable. You save money whether you are paying out of your own funds or using Long-Term Care Insurance benefits to help cover the costs. We already have contracts in place with thousands of credentialed, long-term care providers, so there’s no guesswork or delay in you decision making. Care can begin almost immediately!
In 2004, the average person using the Provider Pathway saved almost $300 per month. Of course, if you receive just a few hours of home health care each week, your savings will be much less. But if many hours of home health care are necessary or if you require facility care, savings can be as high as $800 per month or more!
All providers in our network are instructed to send your bills directly to us. We apply the contracted discount and pay the provider on your behalf. We then reprice the provider’s bill to reflect the discount and include a small service fee. Our repriced bill is sent either to you or to you long-term care insurer, if assignment of benefit is possible. You then have 30 days to pay us the discounted amount with no additional costs.
Home Health Care:
There is a wide variety of services available to make it safer and more comfortable for you to continue to live at home in familiar surroundings. Personal Emergency Response System (PERS):
One important component of a comprehensive home health care program is a Personal Emergency Response System or PERS. A PERS in our home provides for an immediate response in an emergency using a “help button” that activates a signal to the Response Center.
Assisted Living Facility:
Assisted living is a relatively new concept in elder services. It’s a residential community for those who are no longer able to live in their own home and care for themselves. An assisted living community combines the best of all worlds: private living space and communal amenities. Living space varies in size from a studio to a one or two bedroom unit.
Skilled Nursing Facility:
When care at home is no longer feasible and assisted living does not provide the skilled services a chronically ill person needs, a skilled nursing facility or SNF may be the most appropriate level of care. The decision to move into a skilled nursing facility is never an easy one. We work with you to locate the needed facility, obtain the contracted discount, and facilitate the admissions process.
